PRESCRIPTION FRAUD: A 42-year-old Chicago woman who phoned in a prescription for a controlled drug to Walgreens, 5555 W. 79th St., while posing as a health care provider was charged with two felonies May 3 after the medical professional denied making the call, police said. Candace L. Turner, 42, Chicago, was charged with aggravated identity theft/use of identification to commit fraud and obtaining a substance by fraud, police said.

Evergreen Park

ROAD RAGE: The driver of a slow-moving car pointed a gun at a passenger blowing kisses at him in a passing vehicle on May 3 in the 6000 block of Monee-Manhattan Road, according to a complaint to police. The complainant said the driver previously had responded to being followed by tapping his brakes aggressively.

Oak Lawn

SHOPLIFTING BATTERY: An employee at Jewel-Osco, 9424 Pulaski Road, had facial pain and was unable to remember the day’s date after he was punched in the face May 3 while trying to stop a man leaving the store with alcohol in a duffel bag, police said.

ILLEGAL GUN: A 42-year-old Forest Park man who, witnesses said, pointed a gun in random directions and at a stopped school bus was arrested and accused of unlawful use of a weapon and reckless conduct May 10 in the 9700 block of Kostner Avenue, police said. Officers determined Chris C. Landefeld lacked a concealed carry license after recovering an unloaded 9 mm handgun and a USMC knife from his stopped vehicle, police said.

Orland Park

MOTORCYCLIST HURT: A 23-year-old Orland Park man suffered a broken collar bone when he was knocked off his motorcycle April 16 by a 61-year-old motorist from Joliet, police said. Mark R. Lemaire, who swerved into the two-wheeled vehicle, turned himself in April 17 and was cited for leaving a crash site without rendering aid or providing information, police said.

STOLEN CAR: Vehicle surveillance cameras and video from Orland Square mall security cameras led to the recovery of a car stolen out of Maywood and the driver’s arrest, police said in a report released May 8. Officers took Cornelius D. Bass, 35, 100 block of Birch Street, Park Forest, into custody at 10 p.m. March 13 inside the Cheesecake Factory, 138 Orland Square Drive, police said. BURGLARY: Cook County detectives used DNA from a wine bottle to identify a man accused of ransacking a home in unincorporated Rich Township, west of Franciscan Health in Olympia Fields, sheriff’s police said. Investigators extracted the DNA after viewing security video showing a man kicking in the door of the detached garage in the 4000 block of Blackstone Avenue on Sept. 6 and drinking from the bottle after retrieving it from a refrigerator, police said.
