CHAPECOENSE plane crash survivor Rafael Henzel has died of a heart attack aged 45.
The radio reporter was one of the few survivors from the disaster, which occurred due to a pilot error after fuel became critically low.
Henzel was playing football when he died of cardiac arrest, according to reports.
He was one of six survivors from the plane disaster over Medillin, Colombia, that captured the sympathy of the entire footballing world.
The reporter was playing football when he collapsed and was rushed to hospital, but could not be rescued.
A club statement said: “Throughout his brilliant career, Rafael told the story of Chapecoense.
“He was a symbol of the club’s reconstruction and he will always be remembered in the green and white pages of this institution.”
