Tom’s ankle had popped out of place in the stomach churning scenes

This week’s episode will see Dr Ayman and Dr Jehad teaming up to manipulate 20-year-old Tom’s ankle back into position after an accident playing football.

Tom is brought in with a badly dislocated ankle and needs immediate attention.

Whilst being assessed Tom describes what happened, saying: “I was playing football, I landed wrongly on this foot on the side then I heard a crack….then I fell down.

“From 1 to 10 pain is probably 8,9. It hurts like hell. It hurts all the time….I’m bearing the pain all the time.”

Speaking to camera, Doctor Ayman explains: “Serious dislocation and breaks can be very serious. If it is not treated promptly and quickly it can have life changing effects on mobility.

“The longer the joint is dislocated the worse the outcome would be.”

Dr Jehad adds: “He’s got a fractured, dislocated ankle and it is an extremely painful procedure. We will not be able to perform it without any form of sedation.”

Dr Jehad is later seen speaking directly to Tom and telling him: “We can’t do this while you are awake. Promise.

“So, we will need to give you some of the good stuff where we get the canula, we give you some medication we call sedation through your veins, alright?”

The narrator then explains that Tom needs to be sedated with Ketamine for his ankle to be manipulated as his muscles need to relax so that he won’t feel the pain.

Dr Jehad continues: “Manipulating a dislocated joint can be extremely painful.

“When we use conscious sedation it’s a balancing act between saving the patient discomfort and at the same time ensuring that their airway is safe at all times.”

Doctor Ayman revealed how serious it could be if left untreated for too long

A&E After Dark airs tonight on Channel 5 at 9pm.
