Continuity errors are really a huge part of Hollywood. The film and TV production process will always have a few errors throughout, considering there are so many moving parts. Sometimes a continuity error is completely explainable, simple a mistake that was made during the shoot that no one really noticed.
Other times though, that continuity error gains a whole life on its own. It transcends the film and fans even start coming up with theories over what it could really mean, despite the fact that it's really just a mistake that should never have been made. Many classic franchises have big continuity errors, but few have the fan theories that work alongside to try and explain away the problem.
